Person Centered Planning

We at The Arc Fresno/Madera Counties whole-heartedly support the individuals we serve to be active participants in the development and implementation of their life goals and desires while engaging our services.  We are committed to coming to a better understanding of what it takes to deeply support the personhood and common humanity of our brothers and sisters, friends and neighbors who live with a developmental disability. 

All direct care staff are trained on the hallmarks of being person-centered.   And through person-centered planning will learn about the individual’s idea of a good life.  And focus on the support (paid and unpaid) they need.  Our staff will be facilitators and will have certain skills (competencies) that will support a good person-centered planning process.

The Hallmarks of a Person-Centered Approach:

  • A person’s activities, services and supports are based upon his or her dreams, interests, preferences, strengths, and capabilities.
  • The person and people important to them are included in lifestyle planning and have the opportunity to exercise control and make informed decisions.
  • The person has meaningful choices, with decisions based on their experiences.
  • The person uses, when possible, natural and community support.
  • Activities, support, and services foster skills to achieve personal relationships, community inclusion, dignity, and respect.
  • The person’s opportunities and experiences are maximized, and flexibility is enhanced within existing regulator and funding constraints.
  • Planning is collaborative, recurring and involves an ongoing commitment to the person.
  • The person is satisfied with their activities, support, and services.
